Infertility Q & A

by Johannes Ramirez, MD

When is Someone Considered Infertile?

Physicians define infertility as when a woman is not able to become pregnant after having unprotected, frequent sex for six months to a year. That may be due to a single cause in either partner or a combination of factors. Dr. Ramirez wants his patients to know that there are proven, effective, and safe therapies that can significantly increase their likelihood of becoming pregnant.

When Should a Couple Seek Medical Help for Infertility?

In general, a couple should see a doctor if they have been trying on a regular basis to conceive for six months to a year without success.

A woman should consider seeking medical help sooner if she:

  • Is age 35 to 40 and has been trying to conceive for more than six months
  • Is over age 40
  • Menstruates irregularly or not at all
  • Knows she has fertility problems
  • Had been diagnosed with pelvic inflammatory disease or endometriosis
  • Has had one or more miscarriages
  • Had prior treatment for cancer

A man should consider seeking medical help sooner if he:

  • Has a low sperm count
  • Has swelling in the scrotum
  • Has had a vasectomy
  • Has had prior inguinal or scrotal surgery
  • Has small testicles or problems with sexual function
  • Has had cancer or cancer treatment

What Treatments Are Available for Women?

Three common therapies are:

  • Fertility drugs - This is the main treatment when a woman is infertile because of ovulation disorders.
  • Intrauterine insemination (IUI) - Sperm are collected and concentrated and placed directly in the uterus during the time of the month when a woman ovulates.
  • Surgery - Uterine problems such as a uterine septum, endometrial polyps, or intrauterine scar tissue are often treated with hysteroscopic surgery.

What Is In Vitro Fertilization (IVF)?

IVF involves stimulating and retrieving multiple mature eggs from a woman, fertilizing them with a man's sperm in a dish in a lab, and implanting the embryos in the uterus.
